Collect survey responses without a server

Formstork is the backend for your survey and NPS forms. Point any HTML or multi-step form at it, and every response is stored in a searchable dashboard, emailed to you, and ready to export as CSV or JSON.

How it works

When a respondent finishes your survey, their answers POST to Formstork with your access key, and Formstork stores the full submission and emails you a copy. Because it accepts any HTML or JavaScript form on any host, your multi-step survey and conditional questions work exactly as you built them, whether you code the form yourself or assemble it in the no-code form builder. Every response is searchable and taggable in the dashboard, and you can pull the whole set down as CSV or JSON whenever you are ready to analyze it.

Set it up

  1. 1Sign in to Formstork, create a form, and copy its access key.
  2. 2Add your survey as an HTML form, or start from the ready NPS survey form template, then set its action to https://api.formstork.com/submit with your access key in a hidden field.
  3. 3Submit a test response. It lands in your dashboard and inbox right away, with spam filtered out.
  4. 4Optionally connect Google Sheets, Slack, or an autoresponder, and export the results to CSV or JSON whenever you want to analyze them.
