Collect feedback without building a backend

Formstork is the backend for your feedback form. Point any HTML or JavaScript form at it, or drop in a popup widget, and every response lands in a searchable dashboard, in your inbox, and in the tools your team already uses.

How it works

When someone sends feedback, their form POSTs to Formstork with your access key. Formstork stores every submission in a searchable, taggable dashboard, emails you a copy, and can forward it to Slack, Notion, a Google Sheet, and more. There is nothing to host and no database to run: your form stays plain HTML, and Formstork is the backend behind it.

A feedback button in one line

Add one script tag and a floating Send feedback button appears in the corner of your site. Clicking it opens your form in a popup, so the visitor never leaves the page, and every response flows into the same dashboard.

Route and tag by rating

The rules engine can read the rating field and route a poor score straight to the person who should see it. Your dashboard lets you tag and filter responses so real trends do not get buried.

Search, filter, export

Nothing lives only in an inbox. Search across all your feedback, tag it, and export the whole set to CSV or JSON whenever you want to analyze it or share it with the team.

Set it up

  2. 2Point the form's action at https://api.formstork.com/submit and add your access key as a hidden field. That is the whole integration.
  3. 3Every response lands in your Formstork dashboard and your inbox, spam filtered, searchable, and ready to tag.
  4. 4Optionally add a rule to route low ratings to the right person, connect Slack or a Google Sheet, or turn on an autoresponder (paid plans).
